ASSESSMENT- at Home:

Coordinator gathers dental history to discuss your chief complaint
Coordinator gathers medical history and authorization to treat and communicates with a dentist

<  Dentist examines your teeth, gums, tongue, dentures and soft tissues of the mouth

 TREATMENT PLAN AND CARE:

<  Develop Plan for Care, including: restoring teeth; extractions; adjust, reline or make new dentures
<  Provide emergency care and plan for next steps for the appropriate care setting 
<  Assess any need for transport, specialty referral or required treatment in a hospital setting

 FOLLOW-UP AND MONITOR:

<  Revisit to check progress, diet monitoring and counseling
<  Review for medications causing dry mouth and consult with physicians
< Review oral hygiene results and teach caregivers best methods for proper oral home care support


You can count on our reliable staff of in-home health care providers. At A Helping Hand-at Home, we begin by reviewing your needs through discussions with involved family members; when necessary we consult with the physician(s) of the care recipient, social workers, hospitals, or nursing home staff.  We can also provide services in any of these settings.
 

Next, working in conjunction with the family or caregivers, we develop an individualized program to manage your daily needs. We match your needs to the appropriate caregiver, (perhaps an assistant, hygienist or dentist) and bring them to your home to provide and orient family and caregivers with the necessary daily maintenance care process.

After the caregiver has been provided support for continued care in your home,    we emphasize our unique system of continued personalized contacts through telephone check-in and home visits.  Most important, we continually communicate with the caregiver who is providing care for you.
